Sworn Documents for Your OSAP Application
If your OSAP file has been flagged for an affidavit, the fix is usually quick — but it has to be sworn in front of someone OSAP accepts, and it cannot be signed beforehand. OSAP itself lists a notary public as one of the people who can do it, alongside a lawyer and a commissioner of oaths. We are a notary public and a commissioner for taking affidavits, so either way your document is covered.
Most students come in with a form their school’s financial aid office sent them: a parent reporting no income, income that could not be verified with the CRA, or an application being processed without parental information. Bring it unsigned with photo ID and we will swear it while you wait, for $20 per signature.
Deadlines are the real problem in September. We are open seven days a week including weekends, so an OSAP form that lands on a Saturday does not have to wait until Monday.

OSAP Affidavits — What You Should Know
Who OSAP accepts to swear it
This is the question students actually arrive with. OSAP’s own definition of an affidavit says it is sworn and signed in front of a lawyer, a commissioner of oaths who need not be a lawyer, or a notary public — and that you must swear or affirm that the contents are true.
We are both a notary public and a commissioner for taking affidavits, so whichever wording your form uses, the requirement is met in one appointment. The usual rules for any affidavit apply here too: attend in person, bring photo identification, and do not sign the form beforehand.
When OSAP asks for a sworn document
The usual triggers are financial rather than personal: a parent reporting no income for the year; parental income that could not be verified against Canada Revenue Agency records, including non-taxable or foreign income; or an application being processed without information and consent from one or both parents.
Your school’s financial aid office decides which form applies to your file. We do not assess your eligibility or advise on your application — we swear the document they told you to bring.
Use the current year’s form
OSAP forms are tied to an academic year, and the wording changes between years. Download the version for the year you are applying for from the OSAP forms page, or take the copy your financial aid office sent you. Swearing last year’s form is a wasted trip and a wasted fee.
Bring it unsigned. A form that arrives already signed cannot be sworn and has to be printed again — we print free on site if you email it ahead.
If a parent has to swear it too
Some OSAP forms require a parent or another family member to swear part of the document. Whoever is swearing must attend in person with their own valid photo identification, because the commissioner witnesses each signature separately. Each signature is $20.

Can a notary public swear my OSAP affidavit?

How much does an OSAP affidavit cost?
$20 per signature. If the form requires both you and a parent to swear it, that is two signatures. We will confirm the total before anything is signed, and printing is free if you email the form to us.
Can I sign the form before I come in?
No. The commissioner has to witness the signature, so the form must arrive unsigned. If you have already signed it, you will need a fresh copy — we can print one on site at no charge.
Does my parent have to come with me?
Only if the form requires their signature. Whoever is swearing must attend in person with their own valid photo ID, because each signature is witnessed separately. If your schedules do not line up, call 416-951-3301 first and we will explain the options.
