Asked before a car changes hands in the family

Do both of us have to come, the giver and the receiver?

Yes. Form 1157E has a signature line for the recipient and one for the donor above a single sworn block. Each of you swears or affirms and signs at the desk.

Can an uncle, an aunt or a cousin use this form?

Those words are not among the relationships the form prints. ServiceOntario makes the decision, so ask them before you count on the exemption.

What does it cost to have the statement commissioned?

Commissioning starts at $20 for the first document. Two people swear on this form, so we confirm the total before anything is signed.

Does the sworn statement transfer the car?

No. It supports the tax exemption only. The ownership changes at a ServiceOntario centre, in person.

Other sworn papers we take

The fee for every document is on the services and prices page.
